飛安會昨公布復興航空GE235空難座艙對話紀錄,除駕駛收錯油門造成致命事故,專家也認為2名駕駛雞同鴨講,處理流程「太不細膩」,恐怕沒有照標準作業流程處理;民航局更指出,飛機起飛時系統有問題,依照ATR-600型原廠規定不應起飛。根據飛安會公布黑盒子抄本顯示,飛機10時51分43秒在跑道上滑行時,監控駕駛員(PM)劉自忠呼叫「沒有ATPCS armed」(自動起飛動力控制系統未亮),飛機時速約40海浬。根據ATR原廠規定,只要ATPCS未亮、時速未達70海浬,就應中斷起飛,但當時操控駕駛員(PF)廖建宗卻回答「好,繼續起飛」。

飛安會執行長王興中表示,復興航空未將ATR72-600型飛機相關規定寫入飛航操作手冊,但飛安會訪談復航訓練教官,指出ATPCS有異常就不該起飛。復航總經理吳滬生則說,當時由駕駛操控飛機,「我們無法告知當時的決定」。飛機起飛後17秒,儀表顯示2號發動機熄火並有警告聲響,當時飛機爬升至1200呎,廖建宗不到5秒決定解除自動駕駛,同時收回正常的1號發動機;一名不具名的總機師認為實在「太倉促」,飛機爬升階段,單引擎失效,不應先處理引擎問題,最重要的是「維持高度」,不能大轉彎,等爬到安全高度後再進行「返場」。他也說,如果2個引擎都沒了,就該找地方迫降,因為速度和浮力都不夠,不可能重開引擎。桃園機師工會祕書長李定中也質疑,駕駛的決定是否太慌亂、草率?「單引擎可以爬高、正常飛,不用急著關閉。」另外儀表顯示2號發動機熄火,劉自忠也向廖建宗做此一確認,但廖似乎沒有認知到原先收1號機的動作是錯誤的,否則廖會向劉作再一次確認,李定中擔憂2人可能沒有互相確認,尤其斷油門如此重要大事,理論上PF和PM相互間應該一再確認,「或許2人未依SOP」作業。(中國時報),
